Funded by a grant from the Mother Cabrini Health Foundation, this project aims to address deferred care needs for people who are underserved, underinsured, and uninsured who are living in Queens and the Bronx, the New York City areas hardest-hit by the COVID-19 pandemic. This is being achieved by connecting high-risk patients with AIRnyc’s community health workers (CHWs) who can address their social needs and connect them to providers that can address care caps. The project assists people who may have avoided the health care setting during the height of the pandemic, and who did not subsequently return to care.
