New York State Partnership for Patients

The New York State Partnership for Patients (NYSPFP) is a joint initiative of the Healthcare Association of New York State (HANYS) and the Greater New York Hospital Association (GNYHA)—hospital trade associations representing not-for profit and public health care institutions, including large health systems, acute and long-term care hospitals, skilled nursing facilities, hospices, and home care. Collectively, they represent all of the hospitals in New York State. HANYS and GNYHA have been jointly awarded a contract to become New York State’s Hospital Improvement and Innovation Network (HIIN) by the Centers for Medicare & Medicaid Services (CMS).

As the Hospital Improvement and Innovation Network for New York State, NYSPFP provides participating hospitals with intensive technical assistance and performance improvement-focused training and educational opportunities. NYSPFP also measures and evaluates hospital progress related to the CMS-selected focus areas for improvement. NYSPFP works with hospitals to tailor its approach to best meet the needs of individual institutions.


NYSPFP addresses 12 CMS-identified focus areas in order to meet CMS’s goals to reduce hospital-acquired conditions by 40% and preventable readmissions by 20% and to advance better health, better care, and lower costs. The focus areas have been categorized into the following five domains:

-       Nursing Centered Initiatives

-       Infection Prevention Initiatives

-       Building Culture and Leadership:

-       Preventable Readmissions Initiative

-       Obstetrical Safety Initiative


The NYSPFP supports New York State hospitals as they continually strive to provide the safest and finest patient care. Through their strategic partnership, HANYS, GNYHA, and key stakeholder organizations across the state provide resources, cross-cutting improvement strategies, high-quality educational programs, and tailored support to their member hospitals participating in NYSPFP.
