Building upon the success of the Palliative Care Leadership Network (PCLN), GNYHA Foundation received additional funding from the Fan Fox and Leslie R. Samuels Foundation to launch the Palliative Care Leadership Collaborative (PCLC) in early 2012. After the conclusion of the PCLN, GNYHA identified chief barriers to the growth and sustainability of palliative care services in our area. The PCLC aims to further develop, support, and advance palliative care programs in the region. The goals of the PCLC are to 1) develop a standardized approach to the provision of palliative care services, and 2) support hospital teams as they implement best practices that support this standardized approach. The PCLC will implement the collaborative model, which consists of hospital teams agreeing to an evidence-based set of interventions, sharing educational resources through didactic learning sessions, conference calls, data collection, and analysis to measure progress. Hospital palliative care teams will benefit from shared learning and problem solving, as well as from GNYHA’s technical and analytic support and guidance from the PCLN. Additionally, PCLC participation will assist hospitals in complying with the New York State Public Health Law requirements around the newly enacted Pallatiative Care Information Act and Palliative Care Access Act and will position them to seek The Joint Commission’s advanced certification for palliative care.
